During a telephone call with him on Saturday, "his voice sounded weak and he spoke slowly," Moussawi said.
His lawyer Mohammed Jeshi also voiced doubts over official reports on Khawaja's health. "If he is healthy as authorities claim, why then was I not allowed to meet him despite my repeated requests?"
Khawaja was sentenced with 21 other opposition figures on charges of conspiring to overthrow the ruling Sunni monarchy of Bahrain during last year's protests in the Shiite majority state.
The Court of Cassation will hand over its final verdict in their case on Monday.
Catherine Ashton, the chief diplomat for the European Union, last week called for Khawaja's release.
Bahrain has refused an offer by Denmark to take him in.
