It will be published by an imprint of Penguin Young Readers on September 15, 2016 just weeks before the next US presidential election when mother Hillary will be hoping to follow Bill into the White House and become the country's first woman president.
Under the sub-heading "Get Informed, Get Inspired and Get Going!" the book will address issues such as poverty, homelessness, hunger, access to education, gender equality, health, climate change and endangered species, Clinton and Penguin announced.
It will explore solutions to these challenges and share stories of "inspiring kids and teenagers" who are already working to help improve their communities and the planet, the press statement said.
"My hope is that the book will inspire readers to realize that they can start making a difference now, in their own way, for their family, their community, and our world," Clinton said.
The book will be released simultaneously in the United States, Britain, Ireland, Australia, New Zealand and Canada through Penguin Random House, with foreign language editions to follow.
Educated at the universities of Stanford, Columbia and Oxford, the 35-year-old mother of one has had a busy career.
She worked for a management consultancy and a hedge fund, reportedly earned $600,000 as a special TV correspondent on NBC, and is now vice chair of the Clinton Foundation.
That global charity has come under recent controversy over whether any of its considerable donors create a conflict of interest for Hillary as she seeks the Democratic nomination for the presidency.
Chelsea, who shied away from the media as a younger adult, has fervently backed her mother's second bid for the White House.
